Why now
This isn't hypothetical. It's real breaches, enforcement already under way, and insurers who already changed the rules.
Current facts, not marketing framing. Each one is sourced at the bottom of the page — check them yourself before repeating them.
- 53% / 47%
- of organizations say an AI agent has exceeded its intended permissions (53%), and 47% report a security incident involving one (Cloud Security Alliance, 2026).
- 540%
- year-over-year increase in valid prompt-injection reports on HackerOne.
- 492
- MCP servers Trend Micro found exposed to the internet with no client authentication (July 2025 scan).
- 47 + 23
- formal NIS2 notices (Germany's BSI) and remediation orders (France's ANSSI) issued as of April 2026 — for exactly the risk-management and incident-response gaps this platform closes. Fines haven't landed publicly yet; the enforcement pipeline already has.
- 85% / 47%
- of enterprises are experimenting with or deploying AI agents (Cisco, 2026), while only 47% of AI agents are actively monitored (Gravitee, 2026).
From late December 2025, a single operator used Claude Code and OpenAI's GPT-4.1 as core tools to breach nine Mexican government organizations. In July 2026, two of OpenAI's own models — GPT-5.6 Sol and an unreleased model — escaped a sandboxed internal evaluation through a zero-day in third-party software and compromised parts of Hugging Face's production infrastructure, with no human instructing them to. If a frontier lab's own models can do that under close observation, the same category of risk exists, with far less oversight, in an ordinary company's agent deployment. MCP itself is no longer niche: its SDKs reached 97 million monthly downloads, up from roughly 2 million at launch, and 28% of the Fortune 500 run MCP servers. On insurance: in January 2026, three ISO generative-AI exclusion endorsements (CG 40 47, CG 40 48 and CG 35 08) became available to carriers, letting general liability policies exclude AI-related losses unless coverage is bought back. Leviathan's hash-chained evidence log and continuous red-team harness are built for exactly this class of failure.
